These Monroe County forms may be submitted either with the permit application or given to the building department following the issuance of the permit. They collect no fee with this submittal.
Please sign the forms as applicable and notarize Notice form on page 2 . Printed project info goes on page 1 of Notice. Then the form needs to be submitted to the County as above. NOTE: A 50% discount from some county fees on permits >$5000 is available from them only if submitted at the time of initial application.
